You might remember the disaster that took place at the Chernobyl nuclear plant in April of 1986. What is still the largest industrial accident to ever occur in the world took place because of an unauthorized equipment experiment by a group of engineers at the facility in Ukraine. This was a disaster that could have been avoided if only the engineers listened to the sounding alarms and taken the warnings seriously.
